Patient’s referred via a Medicare TEAM CARE ARRANGEMENT

AEP Dietitian Services provide services for patients referred via a Medicare Team Care Arrangement. Individual consultations allow for personal needs and requirements to be considered, ensuring personal dietary goals are met. Our Accredited Practising Dietitian can assist with a range of health and medical conditions including:

• Diabetes Management
• Weight Management
• Osteoporosis
• Nutritional deficiencies
• High Blood Pressure / Cholesterol
• Eating disorders
• Allergy and Intolerance
• Gastro intestinal disorders

Patients referred to AEP Dietician Services via a GP Management Plan (GPMP), with a valid Enhanced Primary Care (EPC) referral will be bulk billed for the services that are conducted from AEP clinic locations, for the number of visits specified on the EPC referral. Home visits incur a gap payment to allow for travel time and expenses.
